Terms and Conditions. These terms and conditions, upon orientation and thorough explanation done by the head nurse or the staff nurses, apply to all Crossway Kidney Care clients and may be amended from time to time. By affixing your signature, we expect your full compliance and you agree to have read and understood these terms and conditions as listed below:

  1. Patients are given their schedule and time slot and strict compliance should be observed. All patients must be here on or before their time slot. If the patient comes after their time slot, he/she will be moved to the last in line and we will deduct the number of minutes late into the total treatment time. If the patient comes after the cutoff time (9 AM for First Shift and 1 PM for Second shift), he/she will be moved to other shifts or the following day/s provided there are slots available. For uninformed and late notification absences the patient shall pay for priming fee and supplies used if the machine was prepared.
  2. For change of schedule, please inform us at least a day before so we can arrange your desired new schedule. Approval will depend if there are slots available. We will allow swapping of schedule to a patient provided they agree to it. Only the head nurse or Asst. Head Nurse has the right to approve changes in patient’s schedule. In any case of emergency situations, please inform us as soon as possible.
  3. Crossway Kidney Care is not an emergency care unit thus we will NOT accept patients who are unstable and we will require patient to proceed directly to hospitals if they are experiencing any untoward signs and symptoms such as difficulty of breathing, chest pains, discomfort they deemed necessary for emergency intervention or any COVID-like sign and symptom. Crossway and crossway staff will not be liable if anything happens to the patient who does not adhere to this policy. Since Crossway Kidney Care is an outpatient-based dialysis center, we only cater stable patients and those who are able to withstand a sitting position.
  4. We encourage our patients to comply with their hemodialysis schedule. For patients who skipped their hemodialysis session three (3) consecutive times and with untoward sign and symptom, we will require them to see their respective nephrologist or proceed to any Emergency Room for further assessment before we reaccept the patient.
  5. Only patients are allowed in the treatment area and folks shall wait in our reception area. The only time that folks can enter the treatment area is when they have been authorized by the staff nurses.
  6. Eating of heavy meal is not allowed inside the treatment area. Patients may only bring light snacks like biscuits or finger foods and bottled drinks preferably with a straw.
  7. There should be at least one (1) non-minor folk to stay in the center for the whole duration of the patient’s dialysis treatment. In case the folk/s needs to go out of the center they should inform the nurse’s station and should leave their contact number if in case there is an emergency and should be back as soon as possible.
  8. If in case the patient will be hospitalized, we will only retain his/her schedule for one month and will be given a new schedule when he/she returns. Prior to discharge from the hospital kindly inform us so we can give you the list of requirements needed to be submitted before we can reaccept the patient.
  9. For payments, we implement Payment First Policy of our Dialysis Package (treatment + Epoetin if any). Additional charges like medications and supplies used during dialysis treatment may be settled before discharge of the patient. Pending Promissory Note (PN) should be settled before dialysis treatment to avoid delays
